Chckvet Promotional Offer Terms

Offer ends July 31st, 2025, and is limited to prospective customers who booked a Chckvet services demonstration by July 31st, 2025. No purchase of Chckvet services is required. Participants may be eligible for a $40 Amazon.com Gift Card, based on their status as determined solely by Chckvet.

Eligibility and Gift Card Distribution

The $40 Amazon.com Gift Card(s) will be awarded digitally to eligible decision makers after completion of the product demo within four (4) weeks of completion of the demo. Chckvet reserves the sole right to determine whether a participant qualifies as a decision maker for the purpose of this offer. Offer is not available to current customers or to prospective customers or individuals that have participated in a Chckvet demo during the prior six (6) months. Offer may not be combined with any other offers and is limited to one (1) gift card per office.

Restrictions and Responsibilities

Recipient is responsible for all taxes and fees associated with receipt and/or use of the gift card as well as reporting the receipt of the gift card as required under applicable federal and state laws. Chckvet is not responsible for and will not replace the gift card if it is lost or damaged, is not used within any applicable timeframe, or is misused by the recipient. Chckvet is not responsible for re-awarding digital gift cards that have expired; please check your spam folder if you have not seen your gift card within five (5) business days after your demo. Recipient agrees to indemnify, defend and hold harmless Chckvet from and against any and all claims, expenses, and liabilities (including reasonable attorneys' fees) arising out of or relating to a recipient's participation in the promotion and/or recipient's acceptance, use or misuse of the gift card.

Additional Terms

Chckvet is not responsible for any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from the recipient's participation in the promotion or receipt or use of the gift card. Modification of Terms

Chckvet reserves the right to modify these terms and conditions at any time. Changes will be effective immediately upon posting. Participants who have already met the requirements prior to any changes will be honored under the original terms.
